Service Dog Application
If you are interested in applying for an ISD service dog, please follow the link below to complete our application. Once your application has been submitted and reviewed, a representative will contact you to inform you on whether or not your application has been approved. If approved, you will be placed on our waiting list.
Illini Service Dogs does not operate on a first come, first served basis. Each year we evaluate our current list of applicants and determine which of those individual's needs best match the skills of our dogs nearing the end of their training. If a match is found, we reach out to those individuals to begin the placement process. Due to a recent increase in demand, we no longer quote estimated wait times for receiving a dog from us. We encourage all applicants to seek out other means of acquiring a service dog while waiting to hear back from us.
Please note that we specialize in training mobility service dogs but can accommodate certain specialized needs on a case by case basis. At this time, we do not train psychiatric support dogs to assist with mental health disabilities such as PTSD or anxiety.
